JOINT COMMISSION PUBLIC NOTICE
If you have a concern or complaint about patient care or safety, please share this directly with those providing the care or services to you. If you do not feel that your concern has been adequately addressed, you may contact a manager of Central New York Infusion Services immediately at 315-424-7027.
Patient safety concerns can be reported to The Joint Commission in any or all of the three ways below:
- Online. Report through www.jointcommission.org. Use the “Report a Safety Event” link in the “Action Center” on the homepage.
- Fax. Submit report to the Joint Commission by fax to 630-792-5636
- Mail. Send a letter to:
